Information for Parents and Guardians of Pediatric Patients
The New York Eye and Ear Infirmary is committed to
providing a safe environment for your child during the hospital stay. To help us protect your child, please follow
the instructions below:
- Do not remove the identification bracelets that have
been placed on your wrist and your child’s wrist or ankle until your child has been discharged from the
hospital.
- Never leave your child unattended. If you need to leave
your child’s room, you must notify a member of the Nursing Staff.
- Never allow anyone to take your child from the room
unless a member of The New York Eye and Ear Infirmary Nursing Staff (identified by Employee I.D.
badge) authorizes the person to take the child.
- Prior to discharge, do not take your child from the
Nursing Unit.
- If another family member will be staying with your
child in your absence, be sure to introduce this person to the Nursing Staff and inform the family member of
the information included in this notice.
- Pediatric patients are discharged only to the parent or
guardian. In the event that individual is unable to pick up the child:
- The parent will complete the form titled “Pediatric
Discharge without Parent/Legal Guardian.” This form identifies the name and relationship of the person
authorized to pick up the child.
- The nurse will verify identity of the authorized person
by a driver’s license, state issued identification card, or passport. If identity cannot be established, the
child will remain on the nursing unit until the parent is available.

The New York Eye and Ear Infirmary ha asumido el
compromiso de proporcionar un entorno seguro para su hijo durante la estadía en el hospital. Para ayudarnos a
protegerle, siga las siguientes instrucciones:

No se quite el brazalete de identificación que le han
colocado en la muñeca, ni le quite a su hijo el brazalete que tiene en la muñeca o el tobillo, hasta que su hijo
haya sido dado de alta del hospital.

Nunca deje solo a su hijo. Si necesita abandonar la
habitación de su hijo, debe notificar a un integrante del Personal de Enfermería.

Nunca permita que nadie retire a su hijo de la
habitación, a menos que un integrante del Personal de Enfermería de The New York Eye and Ear Infirmary
(identificado mediante un distintivo de ID de Empleado) autorice a dicha persona a
retirarlo.

Antes del alta, no retire a su hijo de la Unidad de
Enfermería.

Si otro familiar se quedará con su hijo en su
ausencia, asegúrese de presentar esta persona al Personal de Enfermería y de comunicarle al familiar la información
incluida en esta notificación.

A los pacientes pediátricos se les da de alta únicamente
si está presente uno de sus padres o su tutor. Si dicha persona no puede buscar al niño:

El padre o la madre completará el formulario
titulado “Alta pediátrica en ausencia del padre/tutor legal”. Este formulario identifica el nombre y la
relación de la persona autorizada a buscar al niño.

Un integrante del personal de enfermería verificará la
identidad de la persona autorizada a través de una licencia de conducir, una tarjeta de identificación emitida
por el estado o un pasaporte. Si no se puede establecer la identidad, el niño deberá permanecer en la unidad de
enfermería hasta que se haga presente uno de sus padres.
