Please arrive on time on the day of your surgery. To help you plan your route, you can click onto

list of subway/bus
instructions.
Street parking around The New York Eye and Ear Infirmary is very limited, but there are parking garages nearby.

list of other parking locations. Note that if you are coming by car, there should be one parent or guardian available to tend the child at all times and another person to drive. Please do not bring other children!

Upon arrival, tell Security at the front door that you are here for surgery and ask for the Admitting Office. You will be directed to an area marked
"Reception", where a friendly admitting clerk will start the process.
|
|
|
|
|
|
A registrar will take the family into a private cubicle and confirm all paperwork details.
Please come prepared with:
- Clearance
from pediatrician
- Results of lab tests
- Insurance information including insurance card and referral if necessary from your health care plan
- List of child's medications
- If you are a legal guardian, original court papers
In Admitting, an ID band will be placed on your child's wrist or ankle. Please do not remove this band until after discharge.
