Outpatient Centers |
|
The outpatient centers are the place to start when you need to see a doctor for an eye or ear-nose-throat problem. Please call (212) 979-4192 to make an appointment for a routine visit. You may also request an appointment using our secure, online form and someone will contact you to make an appointment. If you have a problem that requires immediate attention, you may walk in at any time and you will be seen.
After your visit with us, if the doctor tells you to return for a follow up visit, please make the appointment for your next visit at the discharge desk before you leave. |

To
Make Every Visit Easier |
|
Please come in 30 minutes before your appointment time to register for your visit.
Please bring your Medicare, Medicaid and/or insurance cards and identification with you to each visit. All new patients must show identification, preferably picture ID, with proof of address, when you come for your first visit.
If you belong to a managed care plan, a referral from your primary care doctor is necessary.
It is your responsibility to obtain the referral, from your doctor, prior to your visit with us and to bring it with you when you come for your appointment. Please have the referral made out to The New York Eye and Ear Infirmary.
